The Claims Process

When there is an injury in the workplace, all reasonable and necessary medical treatment must be provided to the injured employee, and the injury should be reported to CompSource Oklahoma’s claims department using the Oklahoma Workers’ Compensation Court’s Form 2, also known as the “Employer’s First Notice of Injury”.

There are several ways the employer can submit the First Notice of Injury:
  • TeleClaim

    Call our toll-free Job Injury Phone Line at 1-800-872-7015, where a CompSource Oklahoma representative will take the Form 2 information over the phone, set up a claim in our computer system and send the employer a copy of the Form 2 in the mail to sign and return.

  • Electronically

    Use our online Injury Report Form. After you supply your e-mail address and submit the form, you will receive an e-mail confirming CompSource Oklahoma has received your claim.

  • Fax

    Fax the Form 2 to (405)962-3000, (405)962-3564, or toll-free

  • Mail

What happens next?
The reported claim is assigned to a claims adjuster. The adjuster is your primary contact for the claim and will coordinate all action related to the claim and follow through until the claim is resolved. Both employer and employee will be notified by mail or phone of the name and phone number of the adjuster handling the claim.

Longshore & Harbor Workers’ Compensation Act

If you employ workers engaged in maritime employment, including but not limited to longshore, harbor workers, shipbuilders, those on navigable waters or on adjoining areas, these workers are covered under the Federal Longshore and Harbor Workers’ Compensation Act.
