Use our online Injury Report Form. After you supply your e-mail address and submit the form, you will receive an e-mail confirming CompSource Oklahoma has received your claim.

Call our toll-free Job Injury Phone Line at 1-800-872-7015, where a CompSource Oklahoma representative will take the the loss information over the phone, set up a claim in our computer system and send the employer a copy of the Form 2 in the mail to sign and return.
